One-on-One Support for Adults with Intellectual and Developmental Disabilities in Their Own Home

Program Overview

The Supported Living Arrangement (SLA) program at Opportunity Village is designed to foster independent living skills for individuals in the comfort of their own home or family home. Our dedicated team members work one-on-one with each participant to create a personalized habilitation plan that promotes self-sufficiency and community integration.

The goal is lasting independence — skills that program participants carry into daily life long after their sessions end.

Money Management Training

Home Management Training
  • Laundry and household chores
  • Cleaning the home
  • Meal preparation
Personal Care Training
  • Personal hygiene (brushing teeth, washing hands)
  • Physical fitness (walking, biking, playing interactive video gamesPersonal Care & Fitness such as “Just Dance”)
Community Outings and Recreation
  • Outings to bowling alleys, grocery stores, and recreation centers
Other Services include
  • Medical appointments

Each program participant is matched with a dedicated supported living assistant who works one-on-one to build a consistent weekly routine.

How Habilitation Plans Are Customized

Support is delivered within the community, either in the individual’s or family’s home. We build schedules around the participant’s life, working with your family to find a consistent routine that fits.

The SLA program offers great flexibility in completing each individual’s habilitation plan. For example:

  • Preparing meals
  • Cleaning
Community Integration and Money Management
  • Budgeting for activities

Who This Program Serves

The SLA program is available to adults with intellectual and developmental disabilities who receive Medicaid waiver funding in Nevada. If you’re unsure about eligibility, our intake team can help you figure that out.

Each plan is customized to address the specific goals and preferences of the individual, making the learning process engaging and practical. Once paired with an individual, team members will have a set weekly schedule. While the schedule can accommodate the team member’s availability, consistency is key for effective program implementation.

How soon does the SLA program show results?

Progress varies by individual. Many program participants begin building consistent skills within the first few weeks of regular sessions. Your team member will track goals and update the habilitation plan to reflect progress.
